We’re delighted to share our 2023 results for closing gaps in care: Our nurses, a key part of CircleLink’s care management platform, addressed 90.3% of Gaps in Care! (For patients on our program for six months or more for the gaps tracked*) The nurses also fully closed gaps in care in the majority of these patients. This achievement highlights the effectiveness of our fully-staffed, fully-managed care management program and especially the hard work of the >130 skilled nurses tracked.
