CircleLink Health LLC, announces winning the Gold medal for innovation from The Connecticut Quality Improvement Award Partnership, Inc., Connecticut’s Malcolm Baldridge National Quality Award. Innovative mHealth company uses mobile phones to help patients manage their chronic conditions.
CircleLink Health’s innovation award was for the use of Mobile Health (mhealth) to target underserved diabetic populations in California (Medi-Cal) Medicaid patients and use mobile phones to improve patient’s diabetes control as measured by their HbA1C. Our team developed sophisticated software that captured a diabetic care plan that reflected current best practices in diabetes care to interact with a patient’s cell phone. The interactive program used cloud based technology to automatically manage and intervene in key aspects of a patients diabetes care. A key feature of our system includes an outreach and engagement module allowing the provider to use the portal to communicate directly with the patients through their cell phones. This technology is highly scalable and offers a low-cost and sustainable way to manage diabetes.
